    Project Overview

    Care Track AI is an AI-powered, mobile-first application designed to empower individuals with hypertension. By connecting with wearable devices, the app transforms passive tracking into proactive care, providing real-time insights, trend analysis, and personalized guidance. This tool bridges the gap between daily measurements and infrequent clinic visits, helping users understand their readings and build sustainable, healthy habits.

    My Role: UX Designer, UX Researcher, Product Manager (as a solo project)

    1. The Problem: The Disconnect in Hypertension Care

    For millions of people with hypertension, managing their condition is a daily struggle that often feels like a guessing game. Traditional methods rely on passive, manual tracking, which leads to a significant disconnect. Patients collect blood pressure readings throughout the day, but without context, these numbers are just data points on a spreadsheet. They arrive at their doctor's appointment with a log of readings, but a lack of understanding of the trends and habits behind the numbers. This leads to confusion, frustration, and a missed opportunity for proactive intervention. The core problem was to create an intelligent system that transforms raw data into a clear, actionable narrative, empowering patients to take control of their health.

    2. The Solution: A Proactive Health Companion

    The solution was to build an intelligent platform that acts as a continuous, compassionate health coach. The application's key features and workflow include:

    • Seamless Wearable Integration: Care Track AI connects with popular wearable devices to automatically import blood pressure, heart rate, and activity data, eliminating the need for manual data entry.

    • AI-Powered Insights: The AI engine analyzes trends in readings over time, correlating them with user-input factors like mood, diet, and exercise. This provides powerful insights that go beyond simple numbers, helping users understand the "why" behind their readings.

    • Real-Time Recommendations: The app provides proactive, personalized recommendations, such as a reminder to take a mindful breathing break during a stressful meeting or a suggestion for a quick walk after a meal.

    • Goal Tracking & Motivation: Users can set and track personalized health goals, receiving encouragement and positive feedback to build and maintain sustainable health routines.

    This platform turns isolated data points into a powerful, personalized, and proactive care plan, reducing confusion and fostering a sense of control for the user.

    3. My Design Process: Building Trust Through Clarity and Empathy

    My design process was centered on a critical question: How can I build a tool that feels trustworthy and empathetic, especially for a sensitive and often anxiety-inducing health condition?

    • UX Research: I conducted interviews with newly diagnosed and chronic hypertension patients, as well as healthcare professionals. A key insight was the overwhelming anxiety associated with high readings and the desire for a tool that was calm and reassuring, not alarmist. This led to the design principle of "calm confidence."

    • Information Architecture: The UI was structured to be intuitive and non-intimidating. The dashboard prioritizes a clear, at-a-glance status, followed by trend graphs and a summary of key insights. This hierarchy ensures that users can quickly grasp their health status without feeling overwhelmed.

    • Prototyping & Visual Design: I prototyped a clean, minimalist UI with a calming color palette and accessible typography. The data visualizations were designed to be simple and easy to interpret, using clear labels and icons. The use of soft colors and rounded corners was a conscious choice to create a gentle, reassuring aesthetic. The "Insights" section was designed to be a clear, narrative-based summary, explaining the data in plain English.

    4. The Final Product: A Clear Roadmap to Wellness

    The final product is a highly functional and intuitive application that serves as a valuable companion for anyone managing hypertension. It is a testament to the idea that AI should not replace human expertise but rather augment it. By transforming passive data into actionable insights, it empowers users to engage with their health proactively. The design prioritizes clarity, empathy, and accessibility, making it a trusted and essential tool in the user's health journey.

    Key Learnings & Outcomes:

    • Empathy is a Core Feature: For a health app, the user's emotional state is a critical design consideration. The tone, visual design, and language must all work together to build a sense of calm and control.

    • Simplicity for a Complex Problem: By simplifying complex health data into clear, easy-to-read visualizations and a simple narrative, the app becomes a truly useful tool for a wide range of users.
